We all want to have a say in what happens in our lives especially in our later years. By making plans that cover your future care, lifestyle, health and finances, you’re not only working out what you want, you are also making things easier for those around you. This process is called Advance Care Planning. This workshop will assist you in exploring what matters to you and what your priorities are for the last stage of life;

  • How to have a conversation about your wishes with your family, loved ones and health care providers.
  • What is involved in palliative care and when it should be offered and finally,
  • How to complete documents such as Advance Care Plan, Advance Health Directive, and Enduring Power of Guardianship.

A thought-provoking workshop.  

 

This event is open to City of Gosnells resident aged 55+.
